Giant Uterine Tumour: Doctors at Wockhardt Hospitals Perform Complex Fertility-Preserving Surgery

Mumbai: In a rare and technically complex procedure, doctors at Wockhardt Hospitals, Mumbai Central, successfully removed a giant uterine tumour measuring 28 × 30 cm and weighing 3.1 kg – comparable to a seven-month pregnancy – while preserving the uterus and future fertility of a 33-year-old unmarried woman.

The patient, an African national, presented with a gradually increasing abdominal swelling, which she initially assumed was weight gain.

With no associated pain, menstrual irregularities, or other symptoms, the uterine tumour progressed silently until it extended up to the xiphisternum, prompting detailed medical evaluation.

Rare Uterine Tumour Case Requires Advanced Surgical Planning

Comprehensive investigations confirmed the presence of a massive uterine fibroid – an exceptionally rare presentation at this size.

Managing a uterine tumour of such magnitude is particularly challenging in young, unmarried women, especially when factors such as high body mass index, fertility preservation, and cosmetic outcomes must be carefully balanced.

The surgery was led by Dr Indrani Salunkhe, Consultant – Gynaecology & Obstetrics at Wockhardt Hospitals Mumbai Central, who chose an open myomectomy with a strong focus on conserving the uterus.

“Fibroids of this size are uncommon and demand meticulous surgical expertise. Our priority was the safe removal of the tumour while preserving the uterus and safeguarding the patient’s reproductive and emotional well-being,” said Dr. Salunkhe.

Despite the unusually large size of the uterine tumour, the surgical team opted for a Pfannenstiel (bikini-line) incision, typically used in caesarean sections, to reduce visible scarring.

In a rare adaptation, a vacuum-assisted technique – commonly used during childbirth – was employed to deliver the 3.1-kg tumour safely through the transverse incision.

The patient required four units of blood transfusion; however, the surgery and post-operative recovery were uneventful.

The uterus was fully preserved, wound healing was satisfactory, and cosmetic outcomes aligned with the patient’s expectations.
